Lille Mocha has not had a great life so far
When she came into rescue she had been dumped in a pound very dirty skinny and mattered
She had a huge abscess on her neck that was riddles with Maggots
We then also discovered she had a heart murmur
we have had a full heart exam completed with a specialist and now know the following that is listed under medical results bellow
In a nutshell Mocha will require medication for the rest of her life which if treated well should be many years to come
She will need to be in a home as an inside dog and no long walks ( more of a house dog)
We will also supply her current medication and the superscription for the second one she will need to start

Medical notes
dings
Study quality:This was an excellent study with clear visualisation of all the major cardiac
structures.
Left Ventricle:The left ventricle myocardium is of normal thickness and echogenicity. The
lumen is of normal diameter and there is normal contractility (measured by fractional
shortening).
Right Ventricle:The right ventricle myocardium is of normal thickness and echogenicity. The
lumen is of normal diameter and there is normal contractility (measured by fractional
shortening).
Left Atrium:The LA is moderately enlarged in size. The LA:Ao measures 2:1 (Normal <1.6).
Right Atrium:The right atrium is normal in size and function.
Aortic Valve:The aortic valve is trileaflet, and appears structurally normal.
Mitral Valve:The mitral valve leaflets are thickened with myxomatous degeneration. Both the
anterior/septal and posterior leaflets are affected. The valve coaptation is abnormal and
there is insufficiency across the mitral valve. No valve prolapse or ruptured CT is seen.
Tricuspid Valve:The tricuspid valve is abnormal in appearance. All three (septal, anterior
and posterior) leaflets are thickened. There is marked tricuspid insufficiency noted at m/s
with evidence of pulmonary hypertension (V<2.75m/s, P<30mmHg).
Pulmonic Valve:The pulmonic valve is normal in structure and function. There is no
insufficiency or stenosis present.
Pericardium:The pericardium is normal.
Diagnosis
Stage B2 MMVD with LA enlargement and PAH
Comments
The echocardiogram confirms the presence of Myxomatous Mitral Valve Disease (MMVD).
There is thickening to both leaflets of the mitral valve with a moderate jet of mitral
regurgitation. There is also thickening to the tricuspid valve leaflets and a small jet of
tricuspid regurgitation with evidence of Pulmonary Arterial Hypertension (PAH). There is left
atrial enlargement and hence an indication (based on the EPIC study) to commence
pimobendan (0.25mg/kg PO BID one hour before a meal). The recommended treatment for
PAH is sildenafil (1mg/kg PO BID). A repeat echocardiogram in 3 months time is
recommended to assess the left atrium and if there is a need to titrate sildenafil for the
PAH. Monitoring of RRR and effort at home by the owner should be performed.
