2. Do you own or rent your premises?

3. If renting, do you have landlord permission to adopt a new pet?

4. How many hours a day will your new pet be unattended? We understand that a lot of people work and have other commitments.

5. Do you travel often?

6. If yes, what travel arrangements will you make for your new pet?

7. Have you “puppy proofed” your house? Eg. pot plants up high. Electrical cords stored away etc.

8. Who will be your new puppies primary care giver?

9. Do you currently own any other pets? If yes, please list their species, breed and whether they are desexed below.

10. Have you owned any other pets that may no longer be with you. If yes, please briefly explain what happened to them.

11. Do you, or does anyone in your household smoke?

12. Are you prepared to maintain the worming, vaccination, dental, physical and mental care of your new puppy? Please remember that vaccinations are kept up to date while in my care but if you are adopting a young puppy they may not be fully vaccinated yet due to age. Any vaccinations due after the adoption is the ethical and financial responsibility of the new owner.
